Millions of Americans live with chronic illnesses such as diabetes, hypertension, and heart disease. Yet primary care clinicians often lack structured ways to understand what matters most to patients living with these conditions. Building on a decade of expertise in serious illness communication and high-quality primary care provision, members of our Serious Illness Care Program and Primary Health Care Program are partnering with the Healing Works Foundation to develop a toolkit that helps clinicians and patients set personalized goals for health and well-being. Future work will support access to services and policy changes that enable truly person-centered care.

Chronic illness drives much of the cost and complexity of the U.S. health system. We’re designing solutions to improve how care is delivered—from better conversations in early dementia, to improving screening for children at risk of type 1 diabetes, to helping clinicians engage more deeply with patients’ goals and values. Our cross-cutting experts from our Primary Health Care Program, Serious Illness Care Program, and Implementation teams are working to make chronic care more coordinated, compassionate, and centered on what matters most to each patient.
