Ariadne Labs’ Home Hospital team is evaluating the feasibility of a home hospital model to bring acute medical care to people experiencing homelessness. This work is being conducted in partnership with the New England Center for Homeless Veterans and Mass General Brigham’s Healthcare at Home’s Home Hospital team. This collaborative initiative is funded by the Rx Foundation.
Individuals experiencing homelessness have higher rates of conditions like diabetes, heart disease, and HIV/AIDS that require adherence to treatment plans and, at times, access to acute medical care. Yet, for people experiencing homelessness, a hospital stay may cause them to lose their bed and their place in line for permanent housing.
As the home hospital model continues to spread in the United States, patients experiencing homelessness are excluded from this care due to their lack of secure housing.
Through this project, we aim to understand the feasibility of delivering care to patients experiencing homelessness to fill this gap. Our work will inform future implementation practices to bring high-quality, equitable care to these populations.
