NAVIGATING AGING & ILLNESS

Purposeful care for meaningful living

As the US population ages, the need for compassionate, person-centered care is more urgent than ever. More than 62 million Americans are over the age of 65, and that number continues to grow. With age comes new health challenges — nearly one in three Americans live with a chronic illness, over 7 million with dementia, and millions more with other serious illnesses.

When health needs become complex, the things that bring meaning to a person’s life can get lost in the shuffle of appointments, diagnoses, symptoms, and treatments.

If a patient’s medical needs are met, but they can no longer do the things they enjoy, are we really helping them to live well?

We’re empowering clinicians with the tools, training, and system support they need to help patients and caregivers feel fully seen and fulfilled, regardless of their health status. We’re reimagining health systems to make meaningful, person-centered health care the standard.

The Challenge

Health care systems are very effective at responding to disease, but they don’t always see the person behind the patient. Medical care’s focus on curing disease or treating symptoms can overshadow things like identity, autonomy, and joy that bring meaning to people’s lives.

Clinicians have limited time for meaningful conversations and often lack adequate resources to support them, resulting in care that may not align with a person’s vision of what it means to live well. This disconnect can leave patients and families feeling lost, while clinicians face burnout and stress.

Our Solutions

Our work combines our decade of impact and learnings in serious illness care with expertise in dementia, eldercare, and primary care to put people first in their care through all stages of life.

We’re creating communication tools and care processes to help clinicians guide difficult conversations to build connections, understand what brings meaning to each patient’s life, and care for them in a way that honors who they are. And, we’re creating system-level solutions that make it easier for clinicians to give every patient the best care. The result is more connected, more empathetic care and a better experience for patients, caregivers, and clinicians.

Whether someone is facing a serious illness, managing chronic disease, or navigating dementia, they deserve care that aligns with their vision of living well.
