The Ariadne Labs Home Hospital team is launching a pilot randomized controlled trial to test the acceptability and feasibility of providing acute psychiatric care to patients in their homes. Presently, home hospital programs exclude patients with an active psychiatric condition; this trial will test whether it is possible to provide safe and quality care to patients via home hospital Behavioral Health Home Hospital (BHH). The BHH care model will provide a minimum of twice-a-day in-person nursing to patients, with remote rounding by the BHH Research Psychiatrist and BHH Research Psychologist. Additional clinical care team members will provide care either in person or remotely. Patients admitted and randomized home as part of the trial will need to have a caregiver present. Additional support will be able to be provided by home health aides and mental health specialists. Medication will be dispensed by the clinical care team, 24/7 monitoring will be conducted, and patients will be able to join in-patient therapy groups remotely. The BHH Research Psychiatrist will be a key member of the BHH clinical care team, as they will provide remote hospital/inpatient-level psychological treatment to patients and participate in the research activities around consenting patients to the study.
Job Summary
The Behavioral Health Home Hospital (BHH) Research Psychiatrist will support an innovative care delivery model, Behavioral Health Home Hospital. The program offers a subset of patients an alternative to standard inpatient psychiatric hospital admission, in which patients can receive highly specialized psychiatric-care services in their homes. The BHH Research Psychiatrist will consent and admit patients to the BHH program and provide high-quality psychiatric care to patients in the BHH program. The responsibilities are performed in accordance with Brigham and Women’s Faulkner Hospital’s policies and standards of practice, utilizing thorough and timely electronic documentation and interdisciplinary communications. Work within a multidisciplinary team that includes social workers and psychologists to optimize workflows and enhance patient care. The BHH Research Psychiatrist can expect to have up to 4 patients on service at a given time. This role might include some overnight work to support patients who have urgent and emergent needs. This is an ideal role for someone interested in a temporary position that allows them the flexibility of continuing their current patient load. This is also an ideal role for someone interested in participating in a research trial that will help broaden the treatment opportunities for patients with active acute-psychiatric conditions.
What you’ll do/Key Responsibilities
- Accurately assesses patients’ needs
- Provides comprehensive, quality active psychiatric services to patients as evidenced by timely completion of the admission evaluation and by providing ongoing assessments and treatment, and documenting these as they occur and at least weekly in the progress notes.
- Develop plan of care that adheres to patient needs, safety and diagnosis for optimal patient outcomes. Patients and their families are actively included in the care planning process.
- Oversees the discharge plan and coordinates with the outpatient psychiatric provider to ensure continuity of care. This should be initiated on admission and evaluated on an ongoing basis for optimal patient safety and outcomes (should include the patient, family and multidisciplinary clinical care team and accesses community resources).
- Coordinates and completes all required risk assessments
- Accurately plans interventions for assigned patients
- Orders appropriate consultations and laboratory studies and documents the results of these in the progress notes, per the medical practitioner.
- Provides education from the time of admission and evaluates on an ongoing basis to meet the needs and abilities of patients’ and caregivers’ learning styles. and is appropriate to the care and treatment provided.
- Reconciles medications accurately and timely at admission, discharge, and transfer, and any changes are integrated into the patient’s care plan and documented in the electronic health record.
- Communication and Collaboration
- Meets regularly with patients and with the treatment team.
- Prepares and communicates patient information to appropriate care team members and others as needed via effective communication methods.
- Initiates referrals, as needed, to other disciplines and to appropriate community resources to maximize patient safety and outcomes.
- Obtains and documents informed consent; applies for guardianships if the capacity of the patient is in question.
- Provides and implements care interventions
- Participate in staff meetings, competencies, mandatory compliance training, educational meetings, and research study trainings.
- Provides a leadership role in organizing and providing patient care. Serves as the clinical supervisor of the multidisciplinary treatment team and the supervisor of the psychologist.
- Software is updated by maintaining the latest version of software applications as defined by the IS department.
Qualifications
- The candidate must have a medical degree.
- Eligible for an unrestricted Massachusetts State Medical License
- Board-certified in psychiatry.
- Adheres to all I C.A.R.E. standards.
Experience
- A minimum of one-year inpatient psychiatry
Knowledge, Skills, and Abilities
- Ability to work with various computer software applications and technologies required including Epic, Microsoft Word, Excel, Teams, and Outlook.
- Demonstrate observational skills and the ability to set
- Demonstrate excellent technical, interpersonal, organizational, oral, and written communication skills required.
- Ability to assess accurately the information given to others either verbally or written.
- Ability to assess, diagnose, and prescribe/supervise treatment to assigned patients while conforming to regulatory standards and applicable laws to decrease psychiatric symptoms/disorders.
- Capacity to initiate and maintain positive, respectful, productive, and professional communications with a variety of service providers, individuals, family/significant others, and legal guardians.
- Record of experience and expertise in inpatient care of patients with serious mental illness, inpatient care of patients with forensic involvement, and inpatient care at an intermediate care facility (or equivalent) is desired.
- Competence in maintaining accurate records and exercise discretion in handling confidential information; resolve problems and conflicts in a diplomatic and tactful manner.
*Preference for candidates who already work in the Mass General Brigham system, trial will be conducted at Brigham and Women’s Faulkner Hospital and Brigham and Women’s Hospital.
