CHRONIC DISEASE

Nearly one in three Americans lives with a chronic condition, making illnesses like hypertension, diabetes, hyperlipidemia, or coronary artery disease among the most prevalent in the U.S. These illnesses can significantly impact patients’ quality of life, and they account for a vast majority of U.S. health care spending. Caring for chronic illness is complex, often involving requiring coordination of treatment across multiple care providers, medication management, and regular follow-up care. 

At Ariadne Labs, we are working to create a better chronic care experience for patients, caregivers, and clinicians. We are developing solutions to improve communication in early dementia in primary care, improve screening and follow-up care for children at risk for type 1 diabetes, and help clinicians connect more meaningfully with patients on what matters most to them in their care – all with the goal of making care for chronic illness more person-centered for every patient.
