CoP Webinar | Opportunities and Challenges in Implementing the Serious Illness Care Program

Anna Sandgren, Helene Reimertz, and their team have been working with the Serious Illness Care Program (SICP) in Sweden since 2017. During this webinar, they will share their experiences implementing SICP, implementation strategies they have learned, and how they have adapted the Serious Illness Conversation Guide. They will also discuss findings from their research on patient identification, core elements in serious illness conversations, and organizational readiness to implement SICP.
