Innovating and scaling advanced care at home for critical moments in patients’ lives
Our mission is to optimize whole-body health for all at home. We seek to generate and disseminate the evidence to deliver advanced care at home so that patients can stay at home no matter their health concerns. We are testing Rural Home Hospital and working to build new models of care at home in a variety of settings.
The Challenge
In the US, a hospital stay can be costly – both for the patient and for the health care system as a whole – and exposes patients to additional risks. And for many Americans, access to hospitals is limited. Nearly 80 percent of rural America is deemed medically underserved by the federal government; residents in these areas often must drive long distances in emergencies and for tests and treatments delivered only in hospital settings.
