Our Primary Health Care team works to strengthen primary health care systems within the U.S. and around the world. Our goal is for everyone to have access to high-quality, person-centered care to keep them healthy when they’re well and help them get better when they’re sick.
With partnerships in more than 50 countries, we collaborate with frontline providers, policymakers, multilateral organizations, and others to identify and fix gaps within primary care systems. Our team brings interdisciplinary approaches and deep subject and clinical expertise to create and advance innovative strategies to measure and improve how primary health care systems function. We advise on service delivery integration, implement strategies to introduce and improve population health management practices, and accelerate the uptake of innovative payment models that incentivize health promotion, disease prevention, and team-based, comprehensive care.
FEATURED RELEASE:
The Ariadne Labs | Penda Health partnership designed and piloted BP Sawa, a chronic care management program for Penda patients with hypertension with the goal to develop and implement a hypertension clinical pathway for clinicians and patients to improve hypertension outcomes.
The Challenge
Around the world, primary health care is frequently fragmented and underfunded, compromising its quality. Systems are built to be reactive, relying on individuals to seek out care. Services address immediate symptoms but fail to account for chronic diseases or social factors that influence health. They are often ill-equipped to monitor population- and system-wide trends, making them slow to respond to changing population needs. And, they often overlook marginalized and vulnerable populations, creating barriers and disparities in care.
